DUE TO THE CURRENT COVID-19 VIRUS, I AM OFFERING TELEHEALTH SERVICES TO ALL OF MY CLIENTS. NEW AND ONGOING. THE FEDERAL AND STATE GOVERNMENT HAVE DEEMED THIS SERVICE POSSIBLE FOR ANYONE, THEREFORE YOUR INSURANCE WILL COVER THE COST AND YOU WILL PAY YOUR REGULAR COPAY, CO-INSURANCE OR TOWARD YOUR DEDUCTIBLE
Services may be covered in full or in part by your health insurance or employee benefit plan. Please check your coverage carefully by asking the following questions:
- Do I have mental health insurance benefits?
- What is my deductible and has it been met?
- How many sessions per year does my health insurance cover?
- What is the coverage amount per therapy session?
- Is approval required from my primary care physician?

I also accept self pay. Please note, MANY INSURANCE COMPANIES DO PROVIDE OUT OF NETWORK
benefits as well. If I am not in network with your insurance company, please inquire about those benefits.
Cash, check and all major credit cards accepted for payment. All payments, including co-pays or co-Insurance are due in full at the time of the appointment.
A $25 fee will be added to checks that are returned NSF.
If you miss your scheduled therapy appointment, and you have not notified me at least 24 hours in advance, you will be required to pay $75.00 for the missed session. This fee will not be covered by your insurance carrier. All cancellations are to be made by calling my work number 630-853-9197.
